The questionnaire is designed to be administered no more than three years and no less than three months before an election.
Any public school district in the United States can order the questionnaire.
For school districts with student enrollments of 25,000 or less, the cost is $4,795. There is a large school district surcharge of $1,000 for every 1-10,000 student increment over 25,000.
